General Treatment
Panic attacks can be controlled medically (antidepressants) and through counseling (behavioral therapy). An effective treatment reduces the number and the frequency of attacks, reduces the anxiety and improves the quality of life. If the panic attacks have a particular trigger (reaction to medicine), treatment may not be necessary, these attacks stopping after removing the trigger (stopping medication if approved by doctor).
Sometimes panic attacks continue even after removing the trigger and may cause panic disorder.
Initial Treatment
It depends on the severity of the anxiety attacks, of the fear and anxiety at the thought of another attack, of the agoraphobia associated with panic attacks.
In general, the initial treatment consists of:
1. Counselors:
– behavioral therapy which should focus on changing the way of thinking and behaving (the most efficient therapy in combating anxiety attacks)
– exposure therapy, a sort of behavioral therapy, which is based on facing the object or situation which causes the fear. It is efficient in treating agoraphobia or anxiety related to a certain place or situation.
2. Medical Treatment:
The most used medical treatments for attacks are:
– Selective serotonin reuptake inhibitors: fluoxetine (Prozac), paroxetine. If these drugs are ineffective or cannot be administered because of side effects they may try other antidepressants.
– Antidepressants with mixed neurotransmitter effects, such as Venlafaxine
– Benzodiazepines: Alprazolam (Xanax), diazepam (Valium), clonazepam are prescribed alone or combined with an antidepressant.
Benzodiazepines are used to control rapid and short-term symptoms and may be part of maintenance treatment alone or combined with antidepressants.
Antidepressants used less in treating panic disorder are antidepressants such as Imipramine, Desipramine or clomipramine and monoamine oxidase inhibitors, such as Izocarboxazid.
After starting medication, symptoms begin to improve after a few weeks. If this improvement is not seen in 6-8 weeks, try a higher dose of medicine.
